Overview Comparativo de tratamientos

Each patient's characteristics, as well as the number, size, and position of their fibroids make each case unique and best suited for a specific treatment. Some cases may be solved by means of any of the different treatments, whereas others may be solved only through one of them.

Most women with fibroids do not need surgery and can be controlled by means of hormonal therapies, such as contraceptive pills, which tend to reduce menstrual flow and alleviate the annoyance brought by fibroids that have been causing abundant bleedings.

The Magnetic Resonance Imaging Unit based in Médica Sur is the only institution in Mexico that offers the most innovative and safe procedure for the treatment of fibroids: the Magnetic Resonance guided Focused Ultrasound Surgery (MRgFUS), otherwise known worldwide as ExAblate 2000.

Treatments may be classified into the three following groups:

1. ExAblate 2000 Procedure

This is the most up-to-date and innovative procedure to treat uterine fibroids. It is the only existing non-invasive treatment, which means that it doesn’t require any surgical incision, it doesn’t leave scars, and that no external agents are introduced whatsoever. Moreover, as the uterus is fully preserved, future pregnancy can be possible.

ExAblate 2000 is the application of an ultrasound that, in combination with Magnetic Resonance, burns fibroid cells, thus making their biological activity cease. From there on, the body absorbs the dead cells of the fibroid, and the fibroid starts shrinking in size (like a dried raisin).  All of this results in the disappearance of the annoying symptoms that the fibroid used to cause when it was active biologically. Once a fibroid is treated with ExAblate, there isn’t any possibility that it will grow back again.  New, different fibroids may appear, but never the same ones that have already been ablated.

This procedure is not painful, does not require a hospital stay, and is performed during a three-to-five-hour session. After the session is over, the patient can return home and resume her normal activities in the next days.

Its price is very competitive in comparison with the traditional methods described in the following paragraph. Most important insurance companies cover this procedure in their medical expense or healthcare policies.

2. Surgery

Surgery is the traditional method that gynecologists usually perform when fibroids produce very annoying symptoms.  There are two types of surgery: hysterectomy and myomectomy.

Hysterectomy. This type of surgery consists in the removal of the uterus and represents a radical solution to this condition. This is a major surgery that employs the use of general anesthesia, requires a long hospital stay, and a prolonged time of recovery. It is the only treatment option that completely rules out the future possibility of having children.

Hysterectomy can be performed by making a large incision in the abdomen (like a Caesarean section);  by laparoscopy (by making small incisions in the abdomen), or through the vagina (which doesn't leave any visible scars).

Myomectomy. This type of surgery consists in the removal of fibroids from the walls of the uterus to restitute it to its previous form. It also requires hospitalization and general anesthesia. As with the ExAblate procedure, myomectomy does not prevent the possibility of new, different fibroids from developing in the future.

As with hysterectomy, myomectomy can be performed either by cutting open the abdomen, by laparoscopy, or by surgery through the vagina. In addition to these, there are other myomectomy variations: instead of removing fibroids, they can be eliminated by causing their biological death, as with the ExAblate procedure.

3. Other Treatments

Uterine Artery Selective Embolization. It consists in injecting micro-particles to obstruct the uterine artery that supplies blood to the fibroids, thus stopping their biological activity because of the lack of blood irrigation. As with the ExAblate procedure, no surgery is required and your body absorbs the dead cells. Therefore, the symptoms that an active fibroid used to cause are no longer produced. However, there is no guarantee that new fibroids may not appear.

Embolization involves radiation (i.e., it is a radiological invasive procedure) and entails the possibility of a number of serious risks: premature menopause, serious infections, bleeding, and the embolization (death of cells or tissue because of the lack of blood irrigation) of other areas, such as the uterus.

Alternative Medicine Treatments. There are other alternatives for the treatment of fibroids, through dietary supplements, massages, naturopathy and homeopathy. Until now, there is no scientific evidence proving their efficiency. To determine their efficacy, further medical research would be necessary, but none is yet available.
